Pendaftaran dibuka mulai Bulan Mei sampai dengan Bulan Juli 2011
Pendaftaran dilayani di Kantor SMK Sultan Agung 1 Tebuireng, pada setiap hari kerja, Jam 08.00 - 13.00 WIB (Hari Minggu Libur)
Ketentuan lain bisa dilihat di sekretarian Pendaftaran, atau Kantor STM/SMK Sultan Agung 1 Tebuireng, No Telpon/Fax : (0321) 8866940

    1. Menyerahkan Foto Copy STTB/Ijazah & SKHUN SMP/MTs/yang sederajat, yang telah diligalisir sebanyak 1 lembar
    2. Tahun kelahiran sesudah bulan Juni 1991
    3. Menyerahkan foto terbaru hitam putih ukuran 3x4 = 4 lembar (bukan foto berwarna)
    4. Menyerahkan : Foto Copy KTP Ayah 1 lembar, Foto Copy Ibu 1 lembar, Foto Copy Kartu Keluarga (KK) 1 lembar
    5. Mengisi Formulir Pendaftaran di Kantor SMK Sultan Agung 1 Tebuireng Jombang
    6. Membayar Uang Pendaftara Rp. 40.000,- (Empat Puluh Ribu Rupiah)
    7. STTB/Ijazah yang belum keluar dapat emnggunakan Surat Keterangan LULUS dari SMP/MTs & harus melampirkan foto copy Ijazah SD/MI
    8. Menyerahkan Surat Keterangan NISN dari SMP/MTs
